To submit a claim please follow the simple instructions below:

1. Download your claim document below.
2. Fill out your details on the .pdf document and ensure it is signed. (You can fill it out digitally or scan a copy in)
3. Submit the document in the contact form provided. (Ensure that you submit the form under the correct name)
